Betsy chose direct implants so that she would wake up from surgery with her new breasts already in place. The idea was to move on as quickly as possible. She also nixed the idea of using tissue expanders as too "gruesome." These stretch your skin slowly until pockets are formed and your doctor can just drop in implants.
